2. Fill Out the Intake Forms Online
If you've booked an appointment with us online before, click "Log In." Otherwise, click "First Time."
3. Book Your Appointment
After you fill out and complete the intake paperwork, you will be taken to the calendar page to reserve your date and time.

Digital IDs can be more difficult for some patients to access or carry because it requires a mobile phone.
OG Reeferalz can print a physical ID for you upon approval* to provide an easy and convenient way for you to present a patient ID for dispensaries or caregivers.
New IDs become active the day after your current registration expires.
*Approval can take 24-48 hours or more on weekends and holidays. The office manager will contact you when it is ready.
